There is no long-term contract to sign when you become a Member of USA+. Payment of the one-time enrollment fee and your first month’s Membership entitles you to receive your Membership Card and all of the benefits it provides. If you find within 30 days that you are not satisfied with the benefits, you may request a full refund. Otherwise, you will be charged monthly for your Membership.

What are your rate plans?

The monthly Membership fee of $89.99 (Platinum), $59.99 (Gold) or $29.99 (Bronze) includes the Primary Member, a spouse, and children up to the age of 19 (age 24 if enrolled full-time in college). If an aging parent is living with you and you can claim them as a dependent on your tax return, you can include them as well under your Membership. For common law marriages, a common law spouse included.

Q.	What is covered?
By becoming a Platinum, Gold, Classic, or Bronze Member of USA+, you have access to discounted fees on dentists, prescription drugs, vision care, chiropractors and alternative health care providers, as well as reduced contracted rates from doctors, hospitals and other medical facilities. As a Member, you also receive other benefits and savings that are described in greater detail on this website.

Are pre-existing conditions covered?

As soon as you receive your USA+ Membership Card, you can begin to utilize the savings from any provider in the network. There are no exclusions for pre-existing conditions — remember, this is NOT an insurance policy, with all of the regulations and exclusions that many of us are used to dealing with. You will receive your health benefits and savings right away, regardless of your previous health.

Q.	Can I choose my own dentist?
If you have an Internet connection, you can access provider lists via the provider Websites. You may also request a list of providers in your area or anywhere in the United States by calling USA+ Member Services at 1-800-USA-1187. You can also use the website to nominate your current care provider to become amember of the USA+ Network.

Q.	Is there a waiting period?
You can begin to use your benefits as soon as you receive your USA+ Membership Card. The Card and your Member Handbook describing all benefits and the procedures to use them will be sent to you by US Mail (First Class) within 7 - 10 business days after your enrollment date. Call USA+ Customer Service to register your card and to request all of the information you need to begin saving money with.
